Checklist for Cashless Admission

Identity & Insurance Cards Aadhaar card or voter ID of patient, policyholder ID, and Swasthya Sathi / Health Insurance E-card.

Consultant Doctor Prescription Admission advice slip and prescription issued by the treating doctor detailing the provisional diagnosis.

Pre-Authorization Form Submission Fill out Part A of the pre-authorization form with hospital assistance. Our TPA desk coordinates directly with the insurer.

Approval & Cashless Treatment Once initial approval is received, hospital admission and medical procedures proceed without out-of-pocket stress.

Apply direct, continuous pressure using clean cloth or bandage. Elevate wounded area if no fracture suspected. Do NOT remove large embedded foreign objects. Rush directly to KHHIMS Trauma Bay.
