Original Bill Mandate
Please present your original money receipt or registration slip at Counter 3 or 4 when collecting printed laboratory and imaging reports.
Third-Party Collection
If a family member or representative is picking up reports on behalf of the patient, the original bill slip and patient's ID proof copy must be shown.

Verified signed PDF copies can be sent directly to your registered WhatsApp number or email address upon request after identity confirmation.
